Our New Resident Process

1. Free Tour

Our doors are open for you. We give a full tour of our facility where you can meet the staff, ask questions and get familiar with the community. This is a great introduction in a laid-back setting for potential residents to get all the information needed to make an informed decision for themselves or loved ones.

2. Evaluation

If we are the right fit for you, we then move on to a thorough evaluation of your needs. This evaluation makes sure we can take care of all your needs, such as medication management and any other care needs.

3. Post Move-In Evaluation

With a personalized room set up, we are ready for you to move into your custom living area. We do a follow up evaluation after 30 days to reassess each resident’s accommodation and level of care needed. We want to do our best to adapt to each resident's needs and make Heritage Creek your home.
